29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: (a) Residential Home, Cross Lanes Drive: On or about November 3, 2016, and time prior, the employer did not train the employee on the hazards of working from heights of up to 19ft. 1in. from grade and did not instruct the employee on the procedures to follow for protection. Holstein & Sons Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard Fall Protection Training Requirements, which was contained in OSHA inspection number 1005249, citation number 1, item number 2b and was affirmed as a final order on February 26, 2015, with respect to a workplace located at St. Marks Church in Saint Albans, WV.
