29 CFR 1904.29(b)(2): The employer did not fill out or correctly fill out an OSHA Form 301 or equivalent for each recordable injury or illness. On or about May 24, 2016, the employer did not provide OSHA 301 forms to OSHA for the following cases recorded on the OSHA 300 log for 2016: a. 2016-3, Cutting Lead, 3/1/16 Cutting Dept, sprained left wrist trying to catch a falling polyurethane bun, classification I, days 15 b. 2016-4, Prod. Lead, 3/5/16, Cutting Dept., pulled groin lifting trying to flip a polyurethane bun in the vertical saw cutting area, classification I, days 30 c. 2016-5, Cutter, 3/21/16, Cutting saw#1, Operator was standing by cutter when a forklift driver bumped a skid causing it to strike the standing operator in the ankles, classification H, days 10 d. 2016-8 ,Poly Pouring, 2/27/16, Poly Area, employee got tangled in a drop cord, no classification provided e. 2016-9, Cutting Lead, 3/5/16, Cutting area, employee felt a stabbing pain while turning a bun. Testing showed not tears or damage, no classification provided f. 2016-10, janitor, 4/7/16, PU Pouring, a door from a mold hit him in the back of the leg, classification H, days TBD In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
