Text For Citation: 02 Item/Group: 003 Hazard:
29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by ingoing nip points, rotating parts, flying chips and sparks. On 3/15/16, at the Gainesville facility in the production area, employees are exposed to finger and hand amputations as moving parts, spinning and rotating equipment used at the facility is not guarded. Instances include: a - Pipe Threader #2 b - Pipe Threader #4 c - Lathe #3 d - Lathe #4 e - Lathe #5 The U.S. Pipe and Fabrication (formerly Custom Fab, Inc.) was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 1064509, citation number 1, item number 3 and was affirmed as a final order on 2/18/2016, with respect to a workplace located at 1101 Industrial Blvd, Gainesville, Ga 30504.
