29 CFR 1926.1053(b)(1): Where portable ladders are used for access to an upper landing surface and the ladder's length allows, the employer does not ensure that ladder side rails do not extend at least 3 feet (.9 m) above the upper landing surface being accessed: This violation was most recently observed between 02/17/2016 - 02/19/2016 at a worksite at 232 Main Street, Haverhill, MA, where ladders used to access upper landing surfaces did not extend three feet beyond the landing surface, exposing employees to a fall hazard greater than 45 feet.
