29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control: a) Crimping machines on AN, UMA1, UMA2, LFA, and JFA lines; On or about January 26, 2016 and at time prior thereto, the employer exposed employees to caught in and amputation hazards in that the employer failed to provide training to authorized employees who perform servicing and maintenance on crimpers on the hazardous energy sources present (including electrical and pneumatic) and the methods and means necessary for energy isolation and control. b) Air Leak Testers on AN, UMA1, UMA2, LFA, and JFA lines; On or about January 26, 2016 and at time prior thereto, the employer exposed employees to caught in and struck by hazards in that the employer failed to provide training to authorized employees who perform servicing and maintenance on air leak testers on the hazardous energy sources present (including electrical, pneumatic, and compressed helium gas) and the methods and means necessary for energy isolation and control. c) Endformer (EF-01); On or about January 26, 2016 and at time prior thereto, the employer exposed employees who change out the tubing on the endformer to caught in and amputation hazards in that the employer failed to provide training to authorized employees on the hazardous energy sources present (including electrical and pneumatic) and the methods and means necessary for energy isolation and control. d) Bender 01 and 03 (BD-01 and BD-03); On or about January 26, 2016 and at time prior thereto, the employer exposed employees who change out the jigs on the benders to caught in and struck by hazards in that the employer failed to provide training to authorized employees on the hazardous energy sources present (including electrical and pneumatic) and the methods and means necessary for energy isolation and control. WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1910.147(c)(7)(i), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 316387562, CITATION NUMBER 1, ITEM NUMBER 3 AND WAS AFFIRMED AS A FINAL ORDER ON AUGUST 10, 2012, WITH RESPECT TO A WORKPLACE LOCATED AT 760 WEST VETERAN'S BLVD, AUBURN AL 36830.
