29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous: a) 223 Grand Street, first floor, north side, panel box area, on or about 1-14-16: Employees were operating various tools powered through a quad receptacle box, without a continuous path to ground due to improper wiring. b) 223 Grand Street, first floor, north side, panel box area, on or about 1-14-16: Employees were operating various tools powered through an extension cord that did not have a continuous path to ground due to a missing ground pin. c) 223 Grand Street, first floor, north exterior wall, scaffold area, on or about 1-14-16: Employees were operating various tools powered through an extension cord that did not have a continuous path to ground due to a missing ground pin.
