29 CFR 1904.29(b)(1): The Log of all Work-Related Injures and Illnesses (OSHA Form 300) and/or the Summary of Work-Related Injuries and Illnesses (OSHA Form 300-A), and/or equivalent forms were not recorded in detail required by the respective forms: a) An employee sustained a recordable workplace injury on July 11, 2015 and the employer did not indicate if the case was an injury or illness by placing a check mark in one of the boxes in Column (M) of the OSHA 300 log, on or about July 22, 2015. b) An employee sustained a recordable workplace injury on July 12, 2015. The employer did not properly describe the injury or illness, parts of the body affected, and the object/substance that directly injured or made the person ill and the employer did not indicate if the case was an injury or illness by placing a check mark in one of the boxes in Column (M) of the OSHA 300 log, on or about July 22, 2015. c) An employee sustained a recordable workplace injury on July 12, 2015. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photograph or video evidence of abatement or other written records.
