29 CFR 1904.7(b)(5): The employer did not record an injury or illness that involves medical treatment beyond first aid, on the OSHA 300 Log. You must enter a check mark in the box for cases where the employee received medical treatment but remained at work and was not transferred or restricted: a) On or about September 13, 2011, Conifer Care Communities dba Christopher House, as the exposing employer, did not record the following workplace injuries and illnesses correctly on the OSHA 300 Log for calendar year 2008: On or about August 8, 2008, Entry #804 was recorded only as an injury in Column (M1). The correct recording should also have a check for Other Recordable Cases Column (J). On or about October 19, 2008, Entry #805 was recorded with 1 day in Column (K). The correct recording should be a check in Column (J). b) On or about September 13, 2011, Conifer Care Communities dba Christopher House, as the exposing employer, did not record the following workplace injuries and illnesses correctly on the OSHA 300 Log for calendar year 2009: On or about January 10, 2009, Entry #901 which required medical attention was only recorded as an injury in Column (M1). The correct recording should also include a check in Column (J). On or about February 7, 2009, Entry #902 which required medical attention was only recorded as an injury in Column (M1). The correct recording should also include a check in Column (J). On or about August 3, 2009, Entry #905 which was a needle stick incident, was only recorded in Column (M1). The correct recording should also include a check in Column (J). The employers corrected OSHA 300 form now has this entry renumbered as Entry #904.
