Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that on or about 6/15/92, employees oeprating/riding freight elevator were exposed to the hazard of being crushed between moving elevator car and all of the elevator shaft due to car not having a door or gate with interlock, to be closed before elevator could be oeprated. a) 66 Furman Street, Brooklyn, N.Y. - Freight elevator #2238, 2239 on west side of building. Elevator card were not provided with doors/gates with interlocks. ABATEMENT NOTE: One feasible and acceptable abatement method among others to correct this hazard is to take freight elevator cut off service, install gates/doors as stipulated in ASME/ANSI A17.3 1986 Section 3.47 ASME/ANSI 17.1 1989 Section 111.5 (o), 111.5(c)(1), 111.5(c)(2), 204.4(a) and (b) The National Cold Storage Inc. was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent standard, 5(a)(1), which was contained in OSHA Inspection Number , Citation Number 2, Item Number 3, issued on with Final Order Date of .
