29 CFR 1904.29(b)(1): You must enter information about your business at the top of the OSHA 300 Log, enter a one or two line description for each recordable injury or illness, and summarize this information on the OSHA 300-A at the end of the year. Located at Case Farms, Canton Ohio: On or before March 25, 2015 The employer did not record the following workplace injuries and illnesses correctly on the OSHA 300 log for the calendar year 2015: a. On or about March 24, 2015, a laborer (case 1505) sustained an amputation. Column E did not state where the event occurred. Case Farms Processing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard (name previously cited standard), which was contained in OSHA inspection number 98699, citation number 1, item number 4 and was affirmed as a final order on 12/12/2011, with respect to a workplace located at 1818 County Road 160, Winesburg, Ohio, 44690.
