29 CFR 1910.305(g)(2)(iii): Flexible cords and cables shall be connected to devices and fittings so that strain relief is provided that will prevent pull from being directly transmitted to joints or terminal screws. a.) On or about April 8, 2015, at the above addressed job site, employees were exposed to shock and electrocution hazard while using the Canton Fdy & Massey Co. metal shear that had a 220 VAC flexible cord that had a plug that had damaged outer jacket insulation and lacked strain relief.
