29 CFR 1910.219(f)(3): Sprocket wheels and chains which were 7 feet or less above floors or platforms were not enclosed: (a) Shot Blast - The chain and sprocket that drove the tumbler on the Wheelabrator tumble shot blasting machine, and which had a bottom height of 72 inches above the floor, did not have adequate guarding in that the existing guard did not cover the chain and sprocket at all on the side facing the machine, exposing employees to an in-going nip point, on or about January 11, 2013. (b) Shot Blast - The chains and sprockets for raising and lowering the loader and for opening and closing the door on the Wheelabrator tumble shot blasting machine had no guarding, exposing employees working on the platform at the rotary screen to in-going nip points, on or about January 11, 2013. DOMESTIC CASTING, LLC WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH ADMINISTRATION STANDARD OR ITS EQUIVALENT STANDARD 1910.219(f)(3), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 116215120, CITATION NUMBER 1, ITEM NUMBER 7, AND WAS AFFIRMED AS A FINAL ORDER ON JUNE 1, 2011, WITH RESPECT TO A WORKPLACE LOCATED AT 275 NORTH QUEEN STREET, SHIPPENSBURG, PENNSYLVANIA. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
