29 CFR 1926.651(c)(2): Means of egress from trench excavations. A stairway, ladder, ramp or other safe means of egress shall be located in trench excavations that are 4 feet (1.22 m) or more in depth so as to require no more than 25 feet (7.62 m) of lateral travel for employees. a) Smith & Associates Excavating LLC located at 5786 Inlet Point Lane, Westerville, OH 43081: On and preceding November 16, 2022, the employer did not ensure that an adequate means of egress was provided for an employee working in a residential sewer line trench which was over 6 feet deep and 55 feet in length. The following condition exposed employees to struck-by and caught-in hazards. Smith & Associates Excavating LLC was previously cited for a violation of this Occupational Safety and Health standard 29 CFR 1926.651(c)(2), which was contained in OSHA inspection number 1590812, citation number 1, item number 1-a and was affirmed as a final order on June 17, 2022, with respect to a workplace located at Lot # 11 Hamlet Court, DUBLIN, OH 43017 .
