29 CFR 1910.134(e)(1): The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested, or required to use the respirator in the workplace: (a) Ark Woodworks Inc, at 878 Baseline Place, Brighton, CO: On and preceding 4/4/16, the employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested, or required to use the respirator in the workplace. One employee sprayed Sherwin Williams Sher-Wood Water Conversion Varnish on millwork or other wood products. The employee was required to wear a 3M model 6001 half mask air purifying respirator with organic vapor cartridges and P100 filters. The employee had not received a respirator medical evaluation. This condition potentially exposed the employee to a respiratory hazard.
