29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: (a) Warehouse Building, Bagging Area Palletizer; Lockout or tagout devices were not affixed to each energy isolating device when employee(s) where clearing pallet jams and/or cleaning debris from pallets that had broken inside the palletizer elevator. U.S. Minerals, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(d)(4)(i), which was contained in OSHA inspection number 313016743, citation number 1, item 2 and was affirmed as final order on September 26,2012 with respect to a workplace located at 11000 Baldwin Road, Baldwin, IL.
