29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards were present, or were likely to be present, which necessitated the use of personal protective equipment: a) In Shop 3, On Aug. 4, 2014 and at times prior thereto; a personal protective equipment hazard assessment was not conducted for employees standing, walking, working on top of tanks greater than 4 feet above the floor or next lower level for fall protection which requires the use of personal fall protection, guard rails or a safety net. b) In Shop 3, On Aug. 4, 2014 and at times prior thereto; a personal protective equipment hazard assessment was not conducted for welding in close proximity (5-10 feet) of other welders and grinders. Note: Abatement certification is required for this item.
