29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven (7) feet or less above floors or platforms were not enclosed: (a)Fermentationexperts USA, LLC at 1201 North Ellis Road, Sioux Falls, SD 57107: On or about and at times prior to August 15, 2023, the employer did not ensure the chain and sprocket on the Box Cutter Machine within the Cookie Room was guarded. This condition exposed employee to caught-in and amputation hazards associated with the unguarded drive chain and sprocket. (b)Fermentationexperts USA, LLC at 1201 North Ellis Road, Sioux Falls, SD 57107: On or about and at times prior to August 15, 2023, the employer did not ensure the chain and sprocket on the Hammer Mill return suction discharge equipment on the second floor was guarded. This condition exposed employee to caught-in and amputation hazards associated with the unguarded drive chain and sprocket. Fermentationexperts USA, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.219(f)(3) which was contained in OSHA inspection number 1585438, citation number 1, item number 4e, and was affirmed as a final order on October 3, 2022, with respect to a workplace located at 1201 North Ellis Road, Sioux Falls, SD 57107.
