29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle: At a facility located at 1212 N. Enterprise Drive, De Pere, WI 54115; a) The Hendrick Saw rotary saw blade, used for cutting corrugated polycarbonate, was inadequately guarded to prevent employees from being able to reach the exposed lower portion of the blade from below the metal mesh guard at the infeed side of the saw's table. The metal mesh guard presented an opening with the smallest dimension of approximately 6 inches with a distance of approximately 13.75 inches from the opening in the guard to the operating portion of the saw blade. Additionally, an opening in the metal mesh guard at the saw's control box presented an opening with the smallest dimension of approximately 10 inches with a distance of approximately 25.5 inches from the opening in the guard to the operating portion of the saw blade. b) A Blade Cutter Slitter Table, used for cutting multi-walled polycarbonate, was inadequately guarded to prevent employees from being able to reach the exposed cutter blade from below the metal mesh guard at the infeed side of the cutter's table. The metal mesh guard presented an opening with the smallest dimension being approximately 1.75 inches with a distance of 9.5 inches from the opening in the guard to the slitter's blade cutting path. Additionally, the home position of the cutter's carousel was located outside the metal mesh guard and was an unguarded distance of approximately 12.5 inches from the operator's position to the exposed portion of the blade. The Amerilux International LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(3)(ii), which was contained in OSHA inspection number 1387202, citation number 1, item number 6 and was affirmed as a final order on September 19, 2019, with respect to a workplace located at 1212 N. Enterprise Drive, De Pere, WI 54115.
