29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): A) Centrex Plastics, LLC, doing business as American Plastics, located at 814 W Lima St, Findlay, OH 45840: On or about May 16, 2020, the employer did not ensure that all the steps outlined in 29 CFR 1910.147(d), required for the control of the hazardous energy sources, were performed. Employees do not de-energize and lockout all hazardous energy sources of the Toshiba Plastic Injection Molding Press # 51 (Model: ISF720SI-59A; Serial # 415705) prior to conducting activities such as, but not limited to, cleaning "blow backs" (cleaning plastic from the material well on the press) in a close proximity to the energized electrical heater wires. Employees were exposed to 240 volts electrical hazards. As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lock-out/tag-out device application [1910.147(d)(4)], dissipation of residual energy [1910.147(d)(5)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from electrical hazards while employees were conducting activities such as, but not limited to, cleaning "blow backs" (cleaning plastic from the material well on the press) in a close proximity to the energized electrical heater wires.
