29 CFR 1910.219(e)(1)(i): Horizontal belts which had both runs seven feet or less from the floor level were not guarded with a guard that extended to at least fifteen inches above the belt. On or about June 09, 2017, in the production area, where employees were exposed to a caught-by hazard when operating and walking by threading machine #14 without the belt being guarded. MADDEN BOLT CORPORATION WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD 29 CFR 1910.219(e)(1)(i), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 995002 CITATION NUMBER 1, ITEM NUMBER 5b AND WAS AFFIRMED AS A FINAL ORDER ON 11/27/2015, WITH RESPECT TO A WORKPLACE LOCATED AT 13420 HEMPSTEAD, HOUSTON, TEXAS 77040.
