29 CFR 1910.219(d)(1): Pulleys with parts seven feet or less from the floor or work platform were not guarded in accordance with the requirements specified in 29 CFR 1910.219(m) and (o): (a) Shot Blast - The pulley on the motor, 54 inches above the floor, and the pulley to the gear, 74 inches above the floor, on the side of the Wheelabrator tumble shot blasting machine did not have adequate guarding as follows: the existing guard did not cover the pulleys at all on the side facing the motor and the expanded metal had gotten removed around the face of each of the two pulleys on the side facing away from the motor, exposing the spokes of the pulleys and exposing employees to in-going nip points and a caught-in hazard, on or about January 11, 2013. DOMESTIC CASTING, LLC WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH ADMINISTRATION STANDARD OR ITS EQUIVALENT STANDARD 1910.219(d)(1), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 116215120, CITATION NUMBER 2, ITEM NUMBER 4, GROUP A AND WAS AFFIRMED AS A FINAL ORDER ON JUNE 1, 2011, WITH RESPECT TO A WORKPLACE LOCATED AT 275 NORTH QUEEN STREET, SHIPPENSBURG, PENNSYLVANIA. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
