29 CFR 1910.119(h)(3)(iii):The contract employer did not prepare a record which contains the identity of the contract employee, the date of training, and the means used to verify that the employee understood the training. On or about June 7, 2024, in the HFC-245fa Process Unit, the employer did not effectively implement procedures for properly implementing safe and effective lock-out/ tag-out, specifically block-bleed blind flange side of a closed bleed valve on the D reactor fresh feed system (Valve F105). Turner Industries Group, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.119(h)(3)(iv), which was contained in OSHA inspection number #1575813, citation number 1, item number 3 and was affirmed as a final order on September 7, 2023, with respect to a workplace located at 1300 PPG Drive, Westlake LA, 70669.
