29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated, the person certifying that the evaluation has been performed, the date(s) of the hazard assessment, and, which identifies the document as a certification of hazard assessment: a) Facility: On or about August 08, 2023, and at times prior, the employer failed to conduct the required personal protective equipment (PPE) hazard assessment of the work areas to ensure employees were required to use adequate PPE such as but not limited to safety glasses (eye or face protection) or safety gloves (hand protection) during shipping and receiving activities to protect employees from hazards associated with warehouse operations.
