29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following: Guardrail systems, safety net systems, or personal fall arrest systems: A.) Champion Modular, Inc; production area - On or about 11/20/18, and at times prior thereto, employees were engaged in work activities on the top of the houses without the use of fall protection. Employees were working up to 10-feet above ground level. B.) Champion Modular, Inc.; production area - On or about 1/9/19, and at times prior thereto, employees were engaged in work activities on a catwalk that included, but were not limited to, retrieving building materials without guardrails being in place. The catwalk was approximately 8-10 feet above ground level and was missing guardrails.
