29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposed an employee to injury, was not guarded: a. During the week of July 30, 2015, the employer failed to ensure employees operating the Waterbury #3 Trimmer were protected against contact with the ram by utilizing effective machine guarding. b. During the week of July 30, 2015, the employer failed to ensure employees operating the Waterbury #1 trimmer were protected against contact with the ram by utilizing effective machine guarding. c. On or about July 30, 2015, the employer failed to ensure employees operating the Grob Vertical Band Saw, serial number 798, were protected against contact with the unused portion of the saw blade by utilizing effective machine guarding. Telefast Industries, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.212(a)(3)(ii), which was contained in OSHA inspection number 991341, citation number 01, item number 002 and was affirmed as a final order on October 30, 2014, with respect to a workplace located at 777 West Bagley Road, Berea, Ohio 44017 .
