29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s): a) On or about the week of April 8, 2015, in the forge Shop located at 53 Freedom Rd., Painesville, Ohio 44077, the employer did not ensure that the electrical disconnects, and the hydraulic and gravitational energy sources for the 3 inch National Upsetter, were operated in a manner that isolated the energy sources prior to authorized employees performing work, in that locks were not applied to each energy source nor were the energy source(s) relieved, disconnected, restrained, or otherwise rendered safe during setup and dies changes. b) On or about the week of March 1, 2015, in the forge Shop located at 53 Freedom Rd., Painesville, Ohio 44077, the employer did not ensure that the electrical disconnects, and the hydraulic and gravitational energy sources for the 750 ton HPM hydraulic press, were operated in a manner that isolated the energy sources prior to authorized employees performing work, in that locks were not applied to each energy source nor were the energy source(s) relieved, disconnected, restrained, or otherwise rendered safe during setup and dies changes. c) On or about April 9, 2015, in the forge Shop located at 53 Freedom Rd., Painesville, Ohio 44077, during blade changes, the employer did not ensure that the pneumatic energy sources for 150 ton Maxi Press were operated in a manner that isolated the energy sources prior to authorized employees performing work, in that locks were not applied to each energy source nor were the energy source(s) relieved, disconnected, restrained, or otherwise rendered safe during setup and dies changes. d) On or about April 9, 2015, in the forge Shop located at 53 Freedom Rd., Painesville, Ohio 44077, during blade changes, the employer did not ensure that the electrical disconnects, and the hydraulic and gravitational energy sources for Horizontal Cantilever Band Saw (end), were operated in a manner that isolated the energy sources prior to authorized employees performing work, in that locks were not applied to each energy source nor were the energy source(s) relieved, disconnected, restrained, or otherwise rendered safe during setup and dies changes. e) On or about April 9, 2015, in the forge Shop located at 53 Freedom Rd., Painesville, Ohio 44077, during blade changes, the employer did not ensure that the electrical disconnects, and the hydraulic and gravitational energy sources for Horizontal Cantilever Band Saw (middle), were operated in a manner that isolated the energy sources prior to authorized employees performing work, in that locks were not applied to each energy source nor were the energy source(s) relieved, disconnected, restrained, or otherwise rendered safe during setup and dies changes.
