29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: The employer does not ensure all equipment used at the facility is provided with proper machine guarding, exposing employees to the hazard of amputation hazards. a) This violation was observed on or about November 25, 2014, where a Doall horizontal band saw in the structures area is not properly guarded. b) This violation was observed on or about November 25, 2014, where a Roll-in vertical band saw in the structures area is not properly guarded. c) This violation was observed on or about November 25, 2014, where the Niagra brand hydraulic press brake in the fintube area is not properly guarded. d) This violation was observed on or about November 25, 2014, where the Steelweld brand mechanical press brake in the structures are is not properly guarded. e) This violation was observed on or about November 25, 2014, where the Standard brand hydraulic press brake in the structures is not properly guarded. f) This violation was observed on or about November 25, 2014, where the Pacific brand hydraulic press brake in the structures is not properly guarded. Alfa Laval, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 314341223, citation number 1, item number 2, and was affirmed as a final order on May 25, 2010, with a final abatement date of June 25, 2010, with respect to a workplace hazard at 111 Parker Street Newburyport, MA 01950. Alfa Laval, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 314245945, citation number 2, item number 1, and was affirmed as a final order on August 22, 2011, with a final abatement date of August 31, 2011, with respect to a workplace hazard at 300 Chestnut Street Lykens, PA 17048. Pursuant to 29 CFR 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation describing the steps taken to ensure machine guarding is provided to protect operator(s) and other employees from hazard(s) created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.
