29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: The employer is failing to provide adequate training to employees on the recognition of potential fall hazards and on methods or procedures that may be used to mitigate the risk of falling such as the implementation of various forms of fall protection/restraint systems while working on residential homes where the ground to eave height is greater than 6 feet. The most recent example of this was found on the job site located at 3909 N. 191st Street, Elkhorn, NE 68022 where the employer has untrained employees installing the roof sheathing on a new residential home without the use of fall protection. Instances include but are not limited to: I. Installing sheathing for the roof along the west side of the home while the catch platform was located along the northwest corner of the roofline. Two employees were observed obtaining sheathing from the platform and then carrying it to the south to install it on the exposed rafters. The catch platform is 16 feet long and the roofline was measured at 49 feet which exposed the employees while they worked in the unprotected area (33 feet). The employees had been working on the roof for approximately 1.5 hours. The fall distance was measured at 12.5 feet. ii. Installing rafters on the structure along the north side of the home without fall protection of any type. Three persons were observed installing the rafters and were exposed to both interior and exterior falls. The interior falls were measured at 12 feet and the exterior falls were 12.8 feet. Abatement certification and abatement documentation is required for this violation. The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful. The abatement certification sheet is enclosed with the citations.
