29 CFR 1910.147(d)(6): Prior to starting work on machines or equipment that had been locked out or tagged out, the authorized employee did not verify that isolation and deenergization of the machine or equipment had been accomplished: The employer failed to ensure that authorized employees verified the effectiveness of the energy control measures prior to engaging in their daily servicing and maintenance activities. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
