29 CFR 1910.147(c)(6)(i)(C): Where lockout was used for energy control, the periodic inspection did not include a review, between the inspector and each authorized employee, of that employee's responsibilities under the energy control procedure being inspected: On or about September 12, 2014, the employer did not ensure periodic inspections of the lockout procedures included a review of each authorized employee's responsibilities under the energy control procedures. Employees perform lockout on the Ward Die Cutter Machine during the cleaning of the machine at the end of each shift. Energy sources include, but are not limited to; electrical, pneumatic, and hydraulic. INTERNATIONAL PAPER COMPANY WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD, 29 CFR 1910.147(c)(6)(i), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 315156059 , CITATION NUMBER 01, ITEM NUMBER 003, AND WAS AFFIRMED AS A FINAL ORDER ON OR ABOUT JULY 25, 2011 WITH RESPECT TO A WORK PLACE LOCATED AT 6850 WEST 62ND STREET, CHICAGO, ILLINOIS.
