29 CFR 1926.502(e)(9): Positioning device systems shall be inspected prior to each use for wear, damage, and other deterioration, and defective components shall be removed from service. On August 10, 2014, an employee was exposed to a 90 foot fall hazard while performing painting activities on a communication tower due to the use of a defective positioning device system. The employer did not ensure that the defective positioning device system was removed from service. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
