29 CFR 1926.451(e)(1): A means of access for scaffold platforms more than 2 feet (0.6 m) above or below a point of access, such as portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used. a) Jobsite: East scaffold - On or about 5/20/14, a safe means of access was not provided for an employee working from the supported tubular welded frame scaffold. Employee was performing masonry operations from the 3rd working platform, approximately 20 feet above ground level, and accessed the scaffold via the rungs on the integral prefabricated access frames. b) Jobsite: West scaffold - On or about 5/20/14, a safe means of access was not provided for an employee working from the supported tubular welded frame scaffold. Employee was performing masonry operations from the 4th working platform, approximately 26 feet above the lower level, and accessed the scaffold via the rungs on the integral prefabricated access frames. BC Stucco was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 923004, citation number 1, item number 3, and was affirmed as a final order on 2/13/14, with respect to a workplace located at 713 North 20th Street, Philadelphia, PA.
