29 CFR 1910.23(a)(8): Every floor hole into which persons can accidentally walk were not guarded: a. On or about May 20, 2014, at the kitchen area of the above addressed worksite, a floor drainage had its cover broken, exposing employees to a fall hazard. b. On or about May 20, 2014, at the kopens area of the above addressed worksite, a floor drainage was not kept in place, exposing employees to a fall hazard.
