29 CFR 1910.305(g)(1)(iv)(B): Flexible cords and/or cables were run through holes in walls, ceilings, or floors: On or about April 14, 2014, Rig #1, 110V and 480V nominal electrical cables were run through holes in walls. Employees were exposed to electrical shock hazards in the following instances: 1) In the Change House, an 110V nominal electrical cable used to power the flood light outside the change house was run through a hole in the metal wall. 2) In the Top Doghouse, two 110V nominal electrical cables used to power the lights inside and outside of the doghouse were run through holes in the metal wall. 3) At the Active Mud Tanks, three 480V nominal electrical cables were run through metal piping with protection such as bushings.
