29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator was competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation: a) 300 E. Ayre Street, Newport, DE - Employees are required to use a CAT forklift Model: T-30D, Serial: A-33645 without being provided training and evaluation. The employees use the equipment on a daily basis to move equipment and materials in the shop. This exposed the employees to potential injuries from improper use. The violation was observed on or about March 11, 2014.
