29 CFR 1910.95(d)(1): When information indicated that any employee's exposure equaled or exceed the 8-hour time-weighted average of 85 decibels, the employer did not develop and implement a monitoring program: At the facility on the main shop floor, on November 8 2013, and at times thereto, the employer did not develop and implement an audiometric program to assess exposure of employees to high noise sources including but not limited to grinding, arc gouging, welding, metal fabrication, and machining operations, exposing employees to a hearing loss hazard.
