29 CFR 1910.132(d)(2): The required workplace hazard assessment was not certified as being performed, identifying the person completing the assessment, and written certification of the date of the assessment: On or about February 4, 2014 the workplace assessment, designed to identify personal protective equipment (PPE) needed by employees to prevent injury, was not performed. NOTE: The assessment shall be written, titled, dated and certified by signature. The required documented training shall include the following: 1) When PPE is necessary; 2) What PPE is necessary; 3) Limitations of the PPE; 4) How to properly wear and use the PPE, and 5) Inspection and care of the PPE
