29 CFR 1910.178(l)(4)(ii)(B): Refresher training, including an evaluation of the effectiveness of that training, was not provided to the operator when the operator has been involved in an accident or near-miss incident: On or about January 21, 2014, the employer had not provided refresher training or evaluation to employees who were involved in an accident with a powered industrial truck. PACKAGING CORPORATION OF AMERICA WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD, (1910.178(L)(1)(i)), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER (316077189), CITATION NUMBER 01, ITEM NUMBER 001, AND WAS AFFIRMED AS A FINAL ORDER ON (12/08/2011), WITH RESPECT TO A WORKPLACE LOCATED AT (3200 LAKEWOOD AVENUE EAST POINT, GA 30364).
