29 CFR 1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used. Crossbraces shall not be used as a means of access. a. On or about January 13, 2014 - at the above addressed jobsite, an employee conducting roofing operations from a ladder jack scaffold located on the south side of the building, approximately eighteen (18) feet seven (7) inches above the ground was not provided with an access ladder, thereby exposing the employee to fall hazards. b. On or about January 13, 2014 - at the above addressed jobsite, an employee conducting roofing operations from a ladder jack scaffold located on the west side of the building approximately nineteen (19) feet above the ground was not provided with an access ladder, thereby exposing the employee to fall hazards. JOHN KOMPERDA CONSTRUCTION,INC., WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 1926.451(e)(1), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 313934713, CITATION NUMBER 1, ITEM NUMBER 2 AND WAS AFFIRMED AS A FINAL ORDER ON JUNE 19, 2010 WITH RESPECT TO A WORKPLACE LOCATED AT 9855 W. 151ST STREET, ORLAND PARK, IL 60462.
