29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent): (a) On or about September 11, 2013 the inspection revealed that employees were exposed to serious injury or death from falls to a lower level of over 50 feet. The hazard was located on the powerhouse roof of the John Day dam located near Rufus, Oregon. (b) On or about September 11, 2013 the inspection revealed that employees were exposed to a serious injury from a fall of about 5'7" at turbine generator #7. The Army Corps of Engineers was previously cited for a violation of this occupational safety and health standard , which was contained in OSHA inspection number 96892, citation number 1, item number 4 and was affirmed as a final order on September 15, 2012, with respect to a workplace located at McNary Dam in Umatilla, Oregon. The Army Corps of Engineers was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 682898, citation number 1, item number 1 and was affirmed as a final order on November 26, 2012, with respect to a workplace located at 8010 NW Saint Helens Road in Portland, Oregon.
