29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries: a. On or about September 5, 2013, at the Sinchro 4.0 machine area of the above addressed worksite, a fire extinguisher was placed in an area where the access space was of 13.25 inches, exposing employees to a struck by hazard.
