29 CFR 1910.132 (a): The employer did not make sure that personal protective equipment (PPE) was maintained in a reliable condition wherever it is necessary by reason of hazards or processes in a manner capable of causing injury or impairment in the function of any part of the body through physical contact: (a) Cyclone Drilling, Inc., Drilling Rig 17, API 05-045-19478, Rifle, CO 81650: On and before July 7, 2011 the creating, exposing and correcting employer did not maintain the Sala D621241206 or double leg climbing lanyard, MSA10072475, EZ Stop II, used for fall protection. The lanyard was frayed and torn near the D-ring. This condition exposed employees to fall hazards. (Abatement Note: Abatement certification and documentation are required for this item (see enclose Corrective Action Worksheet).
