29 CFR 1910.219(c)(4)(i): Unguarded shaft ends projected more than one-half the diameter of the shaft: a) On or about June 24, 2013, and at times prior thereto, shafts ends to the right of the DA edger operators station were unguarded. b) On or about June 24, 2013, and at times prior thereto, shaft ends on the roll case on the N side of the DA edger were unguarded and exceeded one half the diameter of the shaft. c) On or about June 24, 2013, and at times prior thereto, shafts were unguarded in front of the operator station of the DA edger. d) On or about June 24, 2013, and at times prior thereto, the shaft ends at the drive roller on the West side of the DA edger were unguarded. e) On or about June 24, 2013, and at times prior thereto, shaft ends on the press roll at the auto edger were unguarded. Note: Abatement certification and documentation ARE required for this item. Clearwater Forest Industries, Inc. dBA Evergreen Forest was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.219(c)(4)(i) which was contained in OSHA inspection number 311580807, citation number 01, item number 003a and was affirmed as a final order on April 15, 2009, with respect to a workplace located at Highway 13 South, Kooskia, ID.
