29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation: (a) Paint Shop - chemical storage area: On or about May 30, 2013, the employer failed to ensure eye or face protection was worn when employees worked with chemicals such as, but not limited to methyl ethyl ketone, a flammable chemical that is both an eye and a skin irritant. (b) Paint Shop - paint booth: On or about August 22, 2013, the employer failed to ensure eye or face protection was worn when employees worked with chemicals such as, but not limited to Epoxy Primer Base Component/MIL-PRF_85582E, Type I, Class C2 (Strontium Chromate), which contains hexavalent chromium. The Har-Conn Chrome Company was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 311783799, Citation Number 1, Item Number 5 and was affirmed as final order on May 25, 2010, with respect to a workplace located in West Hartford, Connecticut.
