Location: 42 West Street, Brooklyn, NY. 5th Floor Elevator Shaft Room. a) Double duplex electrical boxes listed for mounting were not mounted. Employees use the duplex box to control the hoist to move materials. The New Haven Maintenance, LLC was previously cited for violation of this Occupational Safety and Health Standard or its equivalent standard, 29 CFR 1926.403(b)(2), which was contained in OSHA inspection number 313432759, citation 1 item 1a, issued on 12/21/2011, this citation become final order on 02/02/2012.
