29 CFR 1910.1048(h)(2)(i): The employer did not ensure that the protective equipment and clothing that had become contaminated with formaldehyde was cleaned or laundered before its reuse: a) Old Acid Room Area - Employees were exposed to contamination hazards from wearing work boots during formaldehyde loading and not cleaning them before reuse after contamination occurred, on or about September 26, 2012. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
