29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification: a) Thnunderbolt Builders, Inc. at 416 Wind River Drive, Windsor, CO 80550: On or before June 9, 2012, the employer was using a metal 4-plex electrical outlet box on the ground, to power 110 volt hand tools, and this metal outlet box was not certified for such use. A ground fault circuit interrupter was in use. Condition exposed employees to electrical shock hazards.
