29 CFR 1904.29(b)(1): A summary form of all recordable work-related injuries and illnesses (OSHA Form 300A or equivalent), was not completed in the detail required by the regulation. Establishment: On or about June 21, 2011, the OSHA 300 Log for the years 2010 and 2011 did not provide a specific description of recordable injuries and illnesses. The information provided for Column F for each of the following five entries was incomplete in that it did not describe the object or the substance that directly injured or made the employee ill: 2010 OSHA 300 Log: A. On or about May 25, Factory - an employee injury was logged as "finger". B. On or about August 12, Factory - an employee injury was logged as "groin pull". C. On or about October 5, Factory - an employee injury was logged as "lower back". D. On or about December 16, Factory - an employee injury was logged as "knee". 2011 OSHA 300 Log: E. On or about April 29, Shipping - an employee injury was logged as "hurt wrist".
