29 CFR 1910.119(l)(4) If a change covered by this paragraph results in a change in the process safety information required by paragraph (d) of this section, suc h information shall be updated accordingly. The employer does not update process safety information whenever a change occurs. This violation was observed on or about April 2010 whe n it was noted that P&ID 18-E-54-3000 was not updated to reflect the installatio n of PSV-1836 as indicated on the request for change (RFC) for the 2001 throughp ut change. This condition can expose employees to fire and explosion hazards. Pu rsuant to 29 C. F. R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation that it is in compliance with the standard, including describing the steps it is taking to ensure process safe ty information is available for all refinery equipment. CALUMET LUBRICANTS WAS P REVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD WHICH WAS CONTAINED IN OSHA INSPECTION 310249941, CITATION NUMBER 01, ITEM NUMBE R 007, ISSUED ON SEPTEMBER09 24, 2007, AND BECAME A FINAL ORDER ON MAY 28 2008, WITH RE SPECT TO A WORKSITE LOCATED AT 3333 MIDWAY AVE., SHREVEPORT, LA 71109. One metho d of abatement among others is to have someone walk down the unit piping and ver ify what is there and add what is missing.
