29 CFR 1910.178(l)(4)(iii): Refresher training and evaluation. An evaluation of each powered industrial truck operator's performance shall be conducted at least once every three years. On or about July 1, 2025 and thereafter, at an establishment located at 242 South Pearl Street, in Berlin, Wisconsin, in the Fabrication Area, the employer did not ensure that personnel operating Yale #39 forklift, GLC080LINGAE088, S/N: D818V02479C, had been been re-evaluated every three years, as required by the standard.
